Valid government-issued ID — International passport, driver's licence, National ID, or voter's card.
Completed card request form — available at any ProvidusUnity Bank branch or via the online customer portal.
Active Domiciliary account in the relevant foreign currency, if you intend to use the card's dual-currency capability.
